Kia ora everyone, and thank you for the time you have taken to complete our medical practice survey.


We are wanting to set up a low cost medical centre in your area and we are gathering data to see whether or not there is support for our organisation to do this.


Currently we are looking at the following areas:

  • Orewa Marae
  • Auckland (Central)
  • Auckland (West)
  • Auckland (North Shore)
  • Rotorua (Fordlands)
  • Rotorua (Ngati Whakaue)
  • Tauranga
  • Whakatane
  • Hamilton
  • Whanganui
  • Levin 
  • Lower Hutt
  • Porirua


We have already identified some Iwi and Non Government Organisations as potential partners however this does not stop us from developing more in these areas and therefore want to hear from you.

 

Every survey completed goes into the draw for a

$100 shopping voucher from Pak n Sav or Countdown drawn every week.
